*Epinephrine 1:1,000 Ampule: Due to several shortages in different concentrations of Epi, we have been shipped a different type of ampule for the 1:1,000 Epinephrine. Please be advised that there could be the NEW amp beginning to circulate in the field. Please note the color difference of the labels.  They ARE THE SAME CONCENTRATION, just different color label, amp size and the fact the new one will NOT be in a blister wrap. Since there is NO OTHER medications that we carry in an glass ampule, there should be no confusion. To answer the question of why the color change, we have to utilize a different manufacturer.

SUBJECT: Stroke Destination Change

 

June 12, 2017

From the Office of the EMS Medical Director:

On March 3, 2015, the Missouri Department of Health and Senior Services approved theupdated MARCER and Missouri Kansas City EMS Region Time Critical Diagnosis Plan.Shortly thereafter, on March 23, 2015, DHSS also released the Missouri Stroke CenterDesignations. In conjunction with the release of the hospital stroke designations, localEMS agencies were informed that immediate enforcement of stroke transport destinationswould begin.On April 13, 2015, the Office of the EMS Medical Director revised the policy as to wherepatients with suspected acute strokes may be transported. Since that time, the Office hasbeen monitoring this policy as well as any subsequent secondary transports to the Level IComprehensive Stroke Centers. In an effort to provide the best care to our patients, theOffice of the EMS Medical Director has decided to revise the hospital destinationdeterminations policy for suspected acute stroke patients. These changes will be effectiveimmediately.

 

Updated Hospital Destination Determinations for Suspected Acute Stroke Patients

Scenario 1:

Patients presenting with the presumptive signs and symptoms of an acute stroke whosetime since last known normal plus EMS transport is less than 12 hours shall betransported to closest Comprehensive Stroke Center only.

(a)    Transport to a Level I Stroke Center ONLY

 

Scenario 2:

Patients presenting with the presumptive signs and symptoms of an acute stroke whosetime since last known normal plus EMS transport is greater than 12 hours shall betransport to the closest appropriate hospital.

(a) Transport to a Level I Stroke Center is NOT required

 

Exception:

KCFD will ultimately respect the request by a patient for transport to a specific hospital,unless contraindicated by the patient’s unstable clinical condition.

(a) Patient preference will supersede the destination determinations as per the revisedKansas City Time Critical Diagnosis Plan.

(b) Patient/family preference is the highest level of authority and patients/familieshave the right to choose their hospital destination even if it is not in their bestinterest.

(c) If patients/families request to go a destination hospital that is not prepared to offerthe level of care required, they must be given specific information to help themmake an informed decision, including the associated risks of deciding on aninappropriate hospital destination. This discussion must be clearly documented inthe ePCR!

 

Additional Notes:

1) Continue to care for patients as per the Stroke/CVA Management Protocols found inthe Kansas City, Missouri EMS Medical Policies and Protocols.

2) Remember to notify the destination hospital as soon as possible of the impendingarrival of an acute stroke patient, including the Cincinnati Pre-Hospital Stroke Scalefindings, the time since last known normal, and whether or not Thrombolytic ExclusionCriteria exist.

3) Thrombolytic (tPA) Exclusion Criteria:

- Head trauma at onset

- History of recent bleeding, current bleeding, surgery or invasive procedure withinthree months

- Bleeding disorder

4) Stroke Center Destinations Approved by the Office of the EMS Medical Director:

 

Level I Stroke Centers

St. Luke’s Hospital-Plaza

Research Medical Center

University of Kansas Hospital
